Isane's nervous manner can conceal how much responsibility she carries. Medical work demands accurate judgment amid fear, and she repeatedly continues communicating and treating casualties even when facing spiritual pressure far beyond her own.
Her development is tied to inheritance. Unohana's example teaches both compassion and formidable discipline, but Isane must eventually lead through her own temperament. She does not need to imitate her captain's hidden history to preserve Squad 4's purpose.
Isane Kotetsu serves as Squad 4's lieutenant and Retsu Unohana's most trusted aide through much of Bleach.
